Create a practice

One practice, one sign-in to start. You can add colleagues afterwards, each with their own.

Practice name

What you call yourselves. It appears in the header so that on a shared machine it is obvious whose records are on screen. Not a patient identifier, and never attached to one.

Your email

Used to sign you in and to reach you about the account; billing receipts come from the payment provider. It is never joined to a case.

Password

12 characters or more, and no rule about symbols — a long one you can remember beats a short one with a symbol in it that ends up on a note beside the screen.

I understand that Casebook holds no patient names or record numbers, that a date of birth is the one identifier it stores if I enter it — which makes the record protected health information, so if my practice is a covered entity a signed business-associate agreement (BAA) is needed first (ask and we will sign one) — and that keeping the record minimal otherwise depends on the codes I choose.
